Forms and Consular Packet Information Sheets

Upon receiving the approved petition I-600A or Visa 37 from NVC or CIS office, ACIVU sends a consular packet to prospective adoptive parents. Forms and some useful information sheets are in the packet.

1. Letter of notifying approval received by ACIVU and provides GUZ number.

3. Form I-604 (Request for and Report on Overseas Orphan Investigation), the overseas orphan investigation form is not available on a website, but it is included in the consular packet we send to adoptive parents; a PDF version is also available on request.

3. For parents adopting orphans classified under the IR-4 orphan visa category, the DHS Form I-864 (http://uscis.gov/graphics/formsfee/forms/files/I-864.pdf) / I-864A (http://uscis.gov/graphics/formsfee/forms/files/I-864A.pdf), Affidavit of Support Under Section 213A of the Act” is required to complete. Your three most recent years’ tax returns, with all schedules and W-2s attached and proof of employment are required.
