NOTICE OF STANDARDIZATION
OF
"POST
PLACEMENT REPORTS"
OF ADOPTED CHILDREN
Since China began the work of placing out
children for foreign adoption, many Chinese children have been adopted
by foreign families. After the adoption of Chinese children by foreign
families, many adoption agencies have attached great importance to the
follow-up service of the post placement and paid attention to timely feedback
the life and upbringing of the adopted children abroad. However, some adoption
agencies have not relayed any information about the adopted children back
to the Center; others have only submitted photos and a brief account of
the adopted children. In order to further render good service to China’s
placement of children for foreign adoption, and to ensure the legitimate
rights of the adopted children of our country, standardized feedback materials
of the adopted children are necessary. The requirements for the standardization
of the feedback materials are as follows:
A. The contents
of the feedback materials should include:
- The name of the adopted child (the
original Chinese name and present given name), date of birth, recent photos
(at least two), the name of the welfare institute which placed out the
child up for adoption, and the health status of the child at the time of
adoption;
- Adoption parents’ names and address
as well as the name of the adoption organization which processed their
adoption;
- The date the adoption parents came
to China for the adoption, and the document number of "Notice
of Coming to China for Adoption";
- Post Placement Report, the contents
of which should include the most recent information available regarding
the adopted child, i. e., physical stamina, intellectual development, health
status, the treatment for sick and disabled children, the attachment and
adaptation of the child to the family, education and naturalization etc.
B. Special requirements:
- The feedback report should be written
by the social worker who is entrusted by the adoption organization concerned
through family visits.
- At least two post placement reports should
be submitted to the China Center of Adoption Affairs, one within a period
of six months after the child was adopted and brought back home, the other
one within another six months after the first report.
- The feedback materials, apart from written
report and photos mentioned above, may also include audio or video tapes.
All written materials should be translated into Chinese.
- For the convenience of the file, feedback
materials of each child should be compiled separately.
